What Is Selective Laser Trabeculoplasty?

SLT (selective laser trabeculoplasty) is an outpatient laser procedure used to treat open-angle glaucoma. It applies short, low-energy laser pulses to the trabecular meshwork, the microscopic structure in the eye responsible for draining aqueous humor. This stimulation improves the meshwork’s drainage capacity, lowering intraocular pressure without damaging the surrounding tissue.

It’s an established technique with more than 20 years of clinical use, backed by large clinical studies, including the LiGHT trial (2019), which showed that SLT can be as effective as pressure-lowering eye drops in newly diagnosed patients.

Contraindications

SLT isn’t recommended for:

  • Angle-closure or narrow-angle glaucoma (where YAG laser iridotomy is preferred)
  • Active neovascular or uveitic glaucoma
  • Advanced cases requiring direct incisional surgery
  • Cloudy or opaque corneas that prevent visualization of the angle

The Procedure, Step by Step

  1. Pre-procedure evaluation: tonometry, gonioscopy, and optic disc OCT to confirm the indication.
  2. Day of the procedure: topical anesthesia with eye drops. A special lens is placed on the cornea to focus the laser.
  3. Laser application: short, selective pulses to the trabecular meshwork, taking about 10 minutes per eye.
  4. Immediate observation: intraocular pressure is measured right after the procedure. You’re discharged the same day with instructions.

It's called selective because it targets only specific pigmented cells, without damaging the surrounding tissue.

Unlike argon laser trabeculoplasty (ALT, the older technique), SLT uses low-energy pulses of very short duration that avoid thermal scarring of the tissue. That means the procedure can be repeated if its effect fades over time, something ALT didn't allow. SLT is also different from YAG laser iridotomy, which is used specifically for angle-closure or narrow-angle glaucoma.

SLT can be used as initial treatment for open-angle glaucoma (some recent studies support starting with laser before drops), as an add-on when drops aren't controlling pressure, or as an alternative for patients who have trouble using drops every day.
